CLIENT POLICY AGREEMENT and LIABILITY WAIVER

Client Policy Agreement For:

HEALTH

I understand I must keep my dog's vaccinations up to date including Rabies, Bordetella, DHPP, Lepto, and Canine Influenza. I understand my dog is required to be on monthly flea and tick prevention medication. I agree to allow my dog to engage in daily strenuous physical activity. I commit to notifying DOGDRM of any physical limitations or illnesses that would impact my dog's ability to participate in group play.

SAFETY

I understand that per my request, my dog is taken hiking and is allowed to socialize with other dogs off leash. Natural areas may have hazards beyond our control including bramble, burrs, poison ivy, mushrooms, sharp branches/random objects, people, and other dangers. I understand that these are natural environments and may contribute to injuries out of DOGDRM's control. I understand that the very best way to avoid large veterinary costs from unavoidable injuries is to purchase pet insurance. We recommend Trupanion and PetPlan. I understand that my dog must be current on vaccinations including Rabies, Bordetella, DHPP, and Lepto. I understand that all dogs in the care of DOGDRM must be on flea and tick prevention medication and I am responsible for keeping them current each month. I understand that if DOGDRM notices my dog displaying behavior that is dangerous or not conducive to a group environment they will be removed from group play situations for the remainder of their stay. A re-assessment will happen before rejoining for future boarding reservations.

PERMISSION TO TREAT

I authorize emergency medical care to be provided by the aforementioned vet, or an appropriate alternative to be determined by DOGDRM in the event that my regular vet is not available or closer care is required. I will reimburse DOGDRM for any charges related to emergency care. I authorize DOGDRM to administer or seek first aid and resuscitative care and I agree to indemnify and hold harmless DOGDRM for all results thereof.

LIABILITY WAIVER

I have read and signed all points on DOGDRM's Client Policy Agreement and I understand that DOGDRM will endeavor to offer sound, safe, and responsible care for my dog(s). I recognize that I am and will remain responsible for the actions of my dog at all times and I hereby indemnify and hold harmless DOGDRM of any and all claims of injury, expense, cost, or damages caused by my dog while under DOGDRM's care and under my own care as a result of following any instructions given by DOGDRM. Additionally, DOGDRM will act with all due respect and caution in my home and in my absence; I hereby agree to indemnify and hold harmless DOGDRM of any and all claims of damage to my home.
